Vital Records request form and write in black ink with yellow highlighter or red ink "For Genealogical Purposes" at the top of the form and in the box requesting purpose of copy. There are no instructions on the form to do this. The dept. treats the long form like a big secret. You pretty much have to know someone doing genealogy or legal research to know about long form birth certificates in Missouri. The form gets mailed with the fee to Jefferson City and in 2-6 months you may get a copy, though possibly not the long form you requested. In Missouri, you take the standard Missouri Health Dept.
